Telephonic Nurse Care Manager

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Telephonic Nurse Care Manager based in the United States. The Telephonic Nurse Care Manager provides remote, patient-centered nursing support to individuals living with chronic and complex health conditions, including heart failure, COPD, diabetes, and hypertension. Through virtual assessments, remote patient monitoring, and proactive outreach, this role helps patients manage their health safely and effectively from home. You will interpret biometric and interactive voice response (IVR) data, identify changes in clinical status, and develop individualized, evidence-based care plans. Working closely with patients, caregivers, and interdisciplinary care teams, you will promote self-management, coordinate care transitions, and help prevent avoidable hospitalizations and emergency visits. This position combines clinical judgment and compassionate communication with technology-enabled care delivery in a collaborative, fast-paced virtual environment.
